Shady Forbes Terrace is named for its location near the intersection of Shady Avenue and Forbes Avenue.
The earliest usage of the name that I can find is a grant of right of way made on September 27, 1923, by J. M. and Rebeca Shwartz to the Peoples Natural Gas Company, which mentions "Shady & Forbes Terrace."[1]
